2. an application form attached with your passport style photo
3. the original and photocopy of a notarized health certificate
4. the original and photocopy of your admission letter
5. the original and photocopy of JW201 or JW202 form
6. a cover letter form your sponsor, his or her ID card, your diploma, and transcript (all need notarization and authentication)
7. the bank statement of the latest four months of your sponsor. If you won a scholarship, please provide relevant certificates.
8. A printed document checklist.
