S1 residence permit, where to apply

Hello, my husband works in Shanghai, and he already has his residence permit and work visa. In my home country, I was granted a S1 one-entry visa to China. I arrived yesterday to Shanghai and have 30 days to apply for the residence permit. I registered in the police station this morning.

Can you please inform me where can I apply for the residence permit? What documents do I have to submit? Thank you very much for your help! - Victoria

To apply for the residence permit, you could go to the local Public Security Bureau Exit and Entry Adiminstration office.

Here are needed documentments:
1. Completed Application Form of Aliens for Temporary Residence in PRC;
2. Four recent 2-inch color photos (full face without hat) against blue background;
3. Valid passport;
4. Accommodation registration printout from the police;
5. valid certificate of your marriage bond.

1. Where can I access the Application Form of Aliens for Temporary Residence in PRC?
Also, I've read about the health certificate, do I still need it?
