About:Chinese Embassies and Consulates

Give up PH Nationality statement

Hi, Im a Filipino and my Husband is a Chinese national. We are going to register my daughter as a chinese citizen and one the req was to have a Give Up PH Nationality Statement. How do we get it or where to get ite or how make it? can anyone please tell me? Also what is the agreement?
