Visa for China trip for US citizen that originate from India

I am a US citizen and I am traveling with a group from India to China for tourism for 10 days and further going to visit Hongkong and return to India. So how I apply for Multiple entry visa from the US? Where to contact for details as there is no consulate in the the state where I live. Please tell me what are the documents I need to get from travel agent from India to apply for visa in the US. How to get application form etc for download?
