Overstayed 2 times

Hi im a student in china i overstayed two times 1. For one day they let me go without fine.
2. Overstayed for 4 days they let me go without fine , but they made me sign written pledge.
Now im willing to apply for a new visa , So should i write all that details in my application form ?and does at mean to my new entry permit?
