Malaysia Passport - from Tokyo to Shanghai to Italy

Hi ... I have a Malaysia Passport and I am flying from Tokyo to Shanghai and to Italy where I need to stay a night at the hotel. Do I need entry permit for transit?

Returning from Italy to Shanghai and back to Tokyo (without having to leave the airport for 3 hours transit). Do I need a visa for this too?
