Chinese passport renewal. many thanks - Jiao

I wish to renew my Chinese passport - on your official Website it is not clear which form to complete - prior to making an appointment - please advise which is the correct form -

Hi, you need to renew it at the China embassy or consulate. You can check the relevant information from their official website. You can try the Chinese version if you cannot find the information.
