Those documents are needed: your passport and copy of its data page, the application form, a recently-taken passport-style photo, an invitation letter from the hospital, an invitation letter of a duly authorized unit, a guarantee letter from your employer, recent 6-month bank statement, round trip air tickets and hotel booking record.
