Hi, We are planning to send 14 year boy for highschool in China.

School had send JW202 filled forms to us. What other documents required to submit for visa application?

Apart from the JW 202 form, you need to submit an application form attached with the boy's recent photo, the boy's passport and a copy of it, the original and photocopy of the admission notice, the original and photocopy of medical report, and a personal statement. If the boy will go to China alone, his birth certificate, parent's consent letter and copies of passports will also be required.
