How I get China Visa to Study In Shanghai?

I am Mr. Souliphet LAOLOUTENG, From Houaphanh Province, I will go to master study in China. I received inform from Shanghai Ocean University that they decided and gave me scholarship to study Chinese language from September 216 to July 2017 and study major 2017 to 2020 in Shanghai Ocean University. what document should I prepare for my entry permit apply please? Now I have Admission Notice and Application for Study in PRC (JW201), Foreigner Physical Examination form and Passport. Thanks
