Leonila, you should go to the police station which has jurisdiction over your residence in China, and apply for a non-criminal record. Then take your registration form of temporary residence and this record to a local notary public to notarize the certificate. Then take it to the Philippines embassy in PRC to authenticate the document. You can get this paper for free, but you should pay for the notarization and authentication services.
