About the medical report for visa

Hi its me Shashila I want to get a document of medical format to apply working visa for china because hospital is asking about that. Otherwise they won't provide the medical

Shashila, as far as I know, there is no such a form at the website of the embassy of PRC in Sri Lanka, but you can download this form (physical examination record for foreigner) from the official website of the embassy of PRC in Pakistan.
Or you can find this form at this website.
