Well, you can post your application to HK Immigration Department directly. The address is: Receipt and Despatch Sub-unit, HK Immigration Department, 2/F, Immigration Tower, 7 Gloucester Road, Wan Chai, HK. Or you can ask your cousin to submit the application to the department for you.

The application documents include a copy of your passport, a copy of your residence permit in mainland, a financial support document, a filled-out application form attached with your photo, a travel itinerary, and an invitation letter from your cousin. If you want your cousin to be your sponsor, the person should submit a sponsor application form, a copy of HK ID card, and a copy of passport.
