where to get the application form

Thank you so much from KARA:) I really appreciate your reply , it's a very valuable and useful help. We got all the documents and material ready in advance. And we will plan to go there first thing in the morning when they are here in USA. just two quick questions, you mentioned a: complete application form.Where can I get that form? At the office of the council general of PRC  once we get there? Or ,do I need to go toms website to download  the form? How long does it take? Do we get the document Same day? After a week? Is this travel,document for our baby be for 3 years? Please take your time and let know, all your help and advice and time on this ,is truly appreciate and valuable for us.Sincerely, Kara,

You can get the application from the consular office or download one from the website of the consulate.The process takes four working days. If you use urgent service, you can get it on the same day. This travel document is valid for two years.
