Dear Juha, if you want to download the e-mode entry permit application form, you can find it on this website or the official website of the embassy of PRC.
But if you ask about stay permit or residence permit application form, usually, you don’t need to download e-mode version. When you go to the exit and entry administration of the local PSB, you will be provided with the form and you need to fill it out there.
