Carol, your staff needs to prepare the relevant required documents containing the application form, three bareheaded color photo taken within six months and an e-edition, the residence permit in Malaysia (original and copy), the written report about passport loss as well as the Chinese identity certificate (if have). Afterwards, submit them to the embassy of PRC in Malaysia to apply for passport reissue personally.
