About:Chinese Consulate in New York

Proof of life Form for retired Chinese living in America

My wife is 51 years old and is required to summit proof of life to continue receive her retirement benefits from the Chinese Gov. Do we have to have this done at the Chinese Consulate or is there another way because we live 7 hours from New York. Any help with this matter would be appreciated. Thank you.

Chuck, if your wife can't go to the consulate to handle it on herself, she can assign someone to do it. The "agent" should submit his/her valid ID and a notarized power of attorney with the validity of 30 days.

Or she can download the form from the website of Chinese consulate, fill the form and mail it to the consulate. She needs to send a paid envelope inside.

The above information is mentioned on the website of Chinese embassy in USA. But I still suggest you make a call to the consulate to confirm how to do it exactly.
