Thanks, I have another concern, in the application they asked my 1st visit in China, I did but that was way back in early 2000 date of which I cannot remember anymore so can I just state the estimate year and place & purpose of visit? I misplaced my passport with that visa already it may be buried in my stuff in Philippines which I cannot find anymore. I am in Cambodia now and no one to help me find this. You think this will be an issue?

I don't think this can be an issue. You can just fill the blank with an estimated, place and purpose of visit. It's ok if you can't find the old passport.
