Visa application for child

I already have an entry permit to visit China but my 5-year old son does not. How do I apply for one for him? Do I have to send MY passport in as well when I submit his application?
We are looking for a visit to PRC in mid-November.

Hi, David. The process is basically the same as that for applying your entry permit. The differences are: 1. You should sign on his application form; 2. You need to submit his birth certificate and a photocopy of both parents' passports; 3. If his father does not go with you, you may need to submit a consent letter wrriten by the father.
