About:Chinese Consulate in Chicago

I would like to get a 10 year visa so that I may visit China as a tourist many times.

On the four page Form V2013 would I fill in Other for the 2.1 and 2.2 question and ask for a 10 year entry permit?

You can state the intention of applying for a 10-year multiple-entry permit in the blanks right behind the last lines of Item 2.1 and 2.2. If you want to give more details, you can fill in the blank of Item 3.1 on page four.
