is it possible to have a jw202 form without the passport number?

I had to apply for a reissued passport due to a lesser period of validity[2 months].But my consultancy had given my old passport number for jw202.Later my new passport number[reissued passport number] was given for the jw202 form.Now,the consultancy has told me that my jw202 form doesnot have my passport number.Is that possible to have a jw202 form without ones passport number from China?if that is possible,can i get visa without having my passport number in the jw202 form.what will be the further procedures that i should carry on.
