Hey, you should apply for a L type. It requires your passport, finished application form affixed with a recent passport photo, round-trip air tickets and hotel reservation. You need to submit your application to the CVASC branches. There are three in Manchester, London and Edinburgh. The cost is 66 sterlings in London and 99 sterlings at the other two centers.

As for vaccinations, you'd better visit a doctor and listen to his advice. Generally speaking, the places you plan to visit are safe.
