Jeanne, you need to apply from the CVASC office in Surabaya or Jakarta. The office does not accept mailed applications, so you need to use an agency if you can't go there in person. You will need the following papers: your passport, a photocopy of the passport's info page, an application form, a recent bare-headed photo, the original and photocopy of your residence permit in Indonesia, and round-trip flight tickets and hotel reservations. After all the documents are ready, go to the office and submit the application.
