Sachith, you need to apply two entry permits separately, one for HK and the other for mainland China. You can submit your applications to Chinese embassy in your country. However, I personally suggest you submit the one for HK early at least four to six weeks ahead of your trip. If the embassy takes care of your application, then it takes around four working days. If your application was sent to HK, then you need to wait four to six weeks. For mainland entry permit, it takes around four working days.
The bank statement should be recent six months. Except for this, you should submit your passport, filled application forms (ID1003A for HK and 2013 versions for mainland), flight itineraries and hotel reservation.
