Did you contact the cnosular department of Chinese embassy in your country? If not, please try to make a call at（0052-55）56164324 / 56164309 / 56164239 between 16:00 to 18:00 from Monday to Friday.

According to what I know, usually the applicants need to do the physical examination at state-owned hospitals and then have the result certified by their Ministry of Foreign Affairs before they take to Chinese embassy.
