with the below details in mind what type of visa am i required to apply for?

I have been asked to work as a foreign language teacher from Sept. 1st 2015 to June 2016, what type should I apply for as I am assuming it would only be 1 entry? I have never been to China before. Also, is a criminal report disclosure certificate mandatory for application to China?

You should apply for a Z type, which grants you only one entry. However, you can get a temporary residence permit with ths Z type so that you can enter and exit from China freely. The non-criminal record is not a mandatory requirement for applying the Z type, but it will be used for getting the residence pemit.
