What documentation is needed for me to get a tourist visa?

I'm a dutch citizen living and working in the US on a J-1 (no citizen or resident)

Bart, you will need the following documents, including your passport, a copy of the photo page of your passport, a filled-out application form attached with your color bare-headed photo taken within six months, the original and photocopy of your J-1 and round-trip tickets and hotel booking records.
