Signing a VISA application for 16 year old child

I am an American traveling with my American daughter...16 years old... to China for a visit with my son a student at Nanjing University. Should I sign her application with the way my name is on her birth certificate or with my married name now?

S.whit, I think you should sign the same name as it shows on your passport.
