Her passport, completed application form, a recent six months' passport size photo, round-trip air tickets and hotel booking in mainland China are required. And she has to provide her legal status in Malaysia if she applies there. Her valid visa for Malaysia should be ok. Besides, you both should submit your applications to the CVASC branch in Malaysia.
