Muhammad, the form is given by your school after you are accepted. I think you needn't pay for it. However, the cost for X1 and X2 type is the same, 500 Naira for normal service, 9000 for express service and 11000 for rush service.

It includes the admission fees. And you can't buy it from anyone because it is given by the school you applied for. To be exact, the school needs to get the forms from related education department.
