Some Questions About Filling Out the Visa Application, 2013 Version

1. Do I affix my passport photo after I print out the filled out application myself? or does the embassy do it when I apply at the office? Is it fine if the photo is the same as the one in my passport?
2. Where can I find my Local ID/Citizenship number? Is it on my birth certificate?
3. How do I figure out what kind of passport/Travel document type I have? I'm not sure which one I should choose
4. For the place of issue of my passport, do I just put my city? or the city and state?
5. I am working as a hostess at a restaurant and attending college now. Which would I choose as my current occupation? or would I pick other and specify? Would I pick more than one that applies?
6. For the Education section, Is it asking how much education I have fully completed? or what kind of education I am doing now?
7. For the 1.25 part of the application, would I put the US or the city and state where I am from? or would I put the city and Illinois since I will be staying in Chicago for a few days?
8. For the Major Purpose of Visit, which one would I choose if I am just visiting a friend that invited me and we will go and see places together?
9. For section 2.6, is this where I put all of my plans and what places and locations we plan to visit? or is this where I put my friend's address? what if we don't have any plans? is that ok?

You need to affix the photo after you print out of the filled application on your own. I have no idea about the photo's size on your passport, but the photo here should be full face, front view, bareheaded and against a plain light colored background with size 48mm * 33mm.

The ID/Citzen number refers to the SSN number in your country.

For the passport, most of people choose the ordinary type, but the real situation depends on your passport's type.

You are suggested to put the city as well as state for the place of issusing the passport.

For the occupation, you can pick up the "Others" and then write your real situation in the blank.

For the education, it means the one you have completed.

You can just write the US for the blank 1.25.

You can choose the "Tourism" for the major purpose of visit.

For the section 2.6, you are suggested to put all your plans and visiting places if you already have a clear plan. It may help you a lot to get it in the end.

is it ok if the email address isn't in all capital letters or doesn't fit all the way in the blank space?

Kristina, US citizens are not required to provide bank statement. It's just a certificate issued by the bank. Perhaps, you may consult this with your bank. Normally, the applicants are required to provide recent six months banks statement.

Since my friend is inviting me to come to China, what kind of stamp does he exactly need to provide? a personal one or like a business one?

If your friend sends you an invitation letter, I think that you needn't provide the bank statement. May I know whether he is a Chinese or foreigner? If he is a Chinese, he needs to provide a copy of his ID card. If he is a foreigner, he needs to provde copies of his passport information and visa pages.

If so, the bank statement is not needed.
