Jade, you need to provide your passport, filled application form, work permit and a recent passport photo. Since you come to China for the first time, you should provide your financial statement, suc as recent six months banks statement, bir-stamped income tax return form, professional ID or a proof from your employer which tells how long you work at the company and how much you earn.0

What if I cant provide my Income Tax Form because my previous employer never filed us at b.i.r and also I don't have any bank account. Is there any other option?0

If you cannot provide them, you can apply your professional ID or so and write a statement to tell the reasons for not providing the Income tax form and then bank statement.0
