About:Chinese Embassy in Ukraine

Chinese Health Examination form not in Ukrainian

By law, doctors in Ukraine cannot complete a form that only has foreign languages on it.
How can I get a Health Exam form for use in China (the Chinese say it MUST be filled on their form).

They suggest the form in Chinese & Ukrainian, but I cannot find a form like this anywhere!
Do I have to make it myself???

Hi, you may need to contact the embassy for this kind of form. Most of the forms found online are written in Chinese and English. The form is usually provided by the embassy and has certain stype. So you cannot make it yourself.

Thank you for the reply. I will check into that. The agent in Ukraine that I spoke with didn't even mention that, so I wonder if it even exists, or if the agent had no idea to even think of that.
