What would be my Chinese zodiac, element, animal and name according to my date & time of birth?

I am a male, born in India at 12.15PM on the 21st of January 1982. The Indian Standard Time is Greenwich Mean Time plus 5-and-a-half hours. Your birth date is December 27,1981, according to Chinese lunar calendar. Your zodiac sign is rooster. Your element is wood. It's impossible to decide your family name in Chinese. I just find two given names, Bo Fu (博富) and Qing Ying (庆英). Bo Fu is a male name and Qing Ying is a female name.
