About:FAQ of Chinese Visa Application

Line 1.3 Other Name(s), Do I need to put all three?

Do I need to put my previous married name? I did have a passport in that name before. I also had a passport in my maiden name before the first marriage.

Yes, you should list all your former names, especially those used in official documents such as ID card, passport etc.
