What type of application form & what documents I need to ask my friend to apply for me in Kuala Lumpur & how much it cost for 10 days return visa to Shanghai?

Hi, application form V2013 is the correct form you need. Basic documents include your passport, photo attached to application form, round-trip ticket and hotel bookings. Fee is RM 85 for a single entry. If you are visit for business purpose, an invitation letter is required.
