Your passport, application form (ID1003A), a recent photo(48mm×33mm), a copy of passport information page, a proof of travel itineary and other valid documents for your next destination. If your application is handled by Chinese embassy in your country, you will get it on the fourth day after your submission. If it was sent to HK Immigration Department, you will wait for a month.

But once you get a HK transit visa, you still need one more entry if you want to go back to mainland PRC. It is ok if you have double entry.
