Your passport: it should be valid at least six months and have two blank pages.
Application form: 2013 version. You should fill the form first and then print it out.
Photo: a recent six months photo used for passport.

Besides, you should provide the invitation letter, or required round-trip air tickets and hotel booking in mainland China.
