Can I send the application form through email and when the visa is ready I go in person to Bern?

I am from Azerbaijan but I currently live in Geneva, Switzerland. I am traveling to China from the 4th - 13th January 2014. I would like to ask the following questions:

1) Is this the correct formula that I have to fill out?
2) Where do I send the completed form?
3) How long will it take?
4) Where do I have to go to get it?
