Health Check Requirements - any additional restrictions?

I have the Health check form filled out by my doctor but I have just heard about an unusual thing where each page has to be stamped by holding one beside the other and stamping in the middle, and also similar stamping half on photo and half on the health check itself?

Is this true? - and is there a detailed list of this and other requirements anywhere? - as I can not find this info anywhere.
