There is just only one same application form for you to apply all different visas. You can click the application form link on this page to look at it. By the way, the applicants are required to fill it online and print it out. It's said that hand-written forms haven't been accepted anymore.

PS: You can not apply online. If you can't apply in person, you can ask your families, friends or even an agent to submit your application and collect your visa.
