China National Employee Passport Damaged - Need Replacement

Please assist. We have a China National Employee under Work Permit Pass. His Passport was damaged/spoiled during laundry washing Please advise how to have a replacement and what are the documents to submit for his application to issue a New Passport and Fee. What are the working hours. Can you kindly fax the Pass Port Application FORM for his fill-up. Our Fax No. 6284 9529 or advice eService to download the form. Hi, he should visit the embassy in person for passport replacement. The working hours of the consular office: 09:00-11:30, Monday-Friday. Fee is SGD 40 and regular processing takes about 1 month. Following documents are required:
Application form for Chinese passport, photo, a photocopy of old passport, Singapore PR (original and copy).
