medical visa details

I am applying on medical grounds what are the requirements I have an invitation letter from the hospital which I want to visit for treatment.advice me pls

If you are visiting for medical treatment, you can apply for business F type. Invitation letter from the hospital is required. Qualified original introduction letter with the full name, title, date & signature of the issuing person from applicant's company, or a bank statement is required if necessary. You should better contact the embassy directly for details.
