Medical report for the school

hello, I am applying for university in China, what kind of medical report do I have to do? where can I get its form?

Hi, it should be a Physical Examination Record for Foreigner, done at the hopsital that appointed by the Chinese embassy. You can visit the embassy's official website for the form.
