where I need to do medical report

Hello,

I have a job offer and now I was asked to provide medical check report from authorized hospital. Can you kindly tell where I need to do medical examination?

As I know, Both original and copy of the Physical Examination Record including blood, HIV and Syphilis test reports are required and it should be done at a big state-owned hospital;
The list of appointed hospitals is available at the front counter of the embassy. For more information, you can also contact the Consular Section of the Embassy by phone: 370-5-2162861, 2162862 or Fax: 370-5-2162682.
