You have to provide other documents:
1. a fully completed application form, 2 passport size color photos with white or blue background in recent six months.
2. valid passport (at least valid for six months before its expiration and two blank pages), a copy of information page and previous records if any.
3. non-criminal record provided by local police and attested by your foreign affairs ministry.
4. "Invitation Letter of Duly Authorized Unit" or original Confirmation Letter of Invitation
5. a recommendation letter from your local Chamber of Commerce & Industry or your company Registration Certificate (photocopy)
6. a supporting letter from your organization
7. your tax certificate
8. your bank statement
9. If you come for the first time, round-trip air tickets and hotel reservation are needed.
