The Yangtze River trip usually starts from Chongqing and ends in Yichang, so you can take a train or flight to reach Chongqing from Xian to take cruise.
Upon reaching YC, there are bullet trains for Shanghai, so you can take them to reach Shanghai. Then fly to HK.
Hope this can help you.

Take the train from shanghai to xian, then train to chongqing, cruise to yichang, bus or train to wuhan, train to Guangzhou, bus or train to hk
