Visum application form

We want to buy 3 visa for China and come to Brussels next week. We have to bring the three passports, the flight tickets and hotel bookings? Do we need photo's, one of each or is this not necessary? Where can we get the application forms?Can we get them in the embassy at the moment of our visit? Do we have to come twice to Brussels? or can we take it the same morning?
