By which hospital medical form is made? civil or allied hospital?

i am applying for study in china. i have to make my medical form. piease tell me that from which hospital i make my medical form? civil hospital or allied hospital?

Either of the hospitals are ok, but make sure get it to be attested by the Ministry of Foreign Affairs Islamabad.
