Medical report for Z visa

I recently applied for a Z visa from HK and was flatly refused. I went to three different consuls and was told that it is not possible for them to issue a Z visa without and medical report and that they can only accept medical reports done in either HK or the Mainland. I asked around but no one seemed to have heard anything about the need for a medical report.
